Adobe Fillable Form
If the form does not open correctly please read the following.
According to Adobe:
"Microsoft Windows 10 ships with two browsers: Internet Explorer 11 and the new Edge browser.
The Edge browser will be the default browser, and Internet Explorer 11 will be available to support legacy workflows. The new Edge browser will not have any support for ActiveX plug-ins (fillable Adobe Forms). Therefore, the Acrobat/Reader plug-in won't work with Edge.
Google Chrome has issues with Adobe fillable forms, or use just Google.

This is why forms aren't going to behave the same as if you opened them in Reader. It's a big problem and makes forms a lot less useful. All the browsers are going that way.""Make sure you have enabled the correct software for your Cell Phone or Tablet to properly view fillable forms.
Use Internet Explorer 11 or Firefox to open PDFs. Enable the Acrobat/Reader plug-in in Internet Explorer or Firefox or any other browser."
The simplest option is to download the worksheet to your computer, fill in the information, save the file and return as an attachment to firstname.lastname@example.org.
